Obesity is a major risk factor for developing arthritis and leads to increased progression of disease. Obesity puts extra stress on weight bearing joints such as lower back and knees. Joint pain is strongly associated with increased body weight. Obesity also leads to increased inflammation (fat cells creates and releases pro-inflammatory chemicals) in body affecting the immune response.

Step 1: Weight Loss Consultation

Weight loss consultation includes assessment of your health and ruling out metabolic etiology for weight gain. Consultation might be followed by annual physical and routine blood work and/or EKG.

Step 2: Deciding On The Medication

Depending on the co-morbidities (Diabetes type 2), weight loss goals- provider will discuss regarding different treatment options:

Semaglutide(Brand names: Ozempic, Wegovy)
Trizepatide(Brand names: Mounjaro, Zepbound)
Phentermaine (Brand names: Adipex)
Orlistat
Metformin

Step 3: Method Of Getting The Medication

Once the medication has been decided upon and if you meet the criteria for coverage through insurance, prior authorization will be submitted to your insurance. (Note: due to high demand of some of these medications, the medication may not be available). Most insurances have a strict criteria for determining coverage (BMI > 30; some medication requires a diagnosis of Diabetes type 2)

If the medication gets approved via insurance, medication will be dispensed from your pharmacy on a monthly basis
Monthly visits are required for close side effect monitoring, weight loss results, and for dose modification
Each month, the provided will select the appropriate dose depending on weight loss results, tolerance, side effects, etc (Note: You may not need the highest dose)
Each visit will include measuring vitals, biometrics, and dietary counseling
Medication is being prescribed as part of weight loss program and not stand-alone therapy

Note: Not all medical insurance plans have weight loss/anti-obesity medication coverage. If the insurance denies the medication or if your insurance does not have weight loss coverage, alternative options are available.

Alternative Option:

Our clinic uses a reputed compound pharmacy for semaglutide and trizepatide.

Weekly visits are required for 1 month to learn how to administer the medication and for close side effect monitoring
After 1 month: during each monthly visit, you will be given 4 (or 5) weekly injections

Internal Medicine

Dr. Mukund Das graduated with a bachelor’s degree in Molecular Biology and Biochemistry from Rutgers University in 2009. He completed his medical school in 2016 from St. George’s University School of Medicine. He then completed his residency in internal medicine at New York Presbyterian- Brooklyn Methodist Hospital in 2019.

Since then, Dr. Das has worked at multiple hospitals in NY, NY, MD, MA, ME where he gained a wide range of clinical exposure. He also worked in an internal medicine clinic in Manhattan, NY. While treating various life threatening conditions at the hospital such as: hypertensive emergency/urgency, diabetic ketoacidosis/ uncontrolled hyperglycemia, decompensated heart failure, COPD and Asthma exacerbation, various arrhythmias, various infectious diseases, etc- he witnessed the lack of emphasis in modern medicine on preventive medicine, patient education, and re-enforcement of medication compliance.

He strongly believes that when a doctor takes their time to educate patients regarding their disease process and treatment options- medication compliance increases. The goal of primary care medicine should be to prevent a patient from being hospitalized by early detection and management of diseases. Dr. Das recognizes the unique health challenges that many patients currently face and his goal is to build a model of primary care that will reduce illness and improve health quality for his patients.
